
On 1 June 1974, a huge vapour cloud explosion occurred at the Nypro chemical plant near the village of Flixborough. One of the worst onshore industrial accidents in UK history. It may be an old one but many of the lessons learned are still relevant today.
While there is (quite rightly) a lot of focus on the role of management of change in the accident, other areas to explore include commercial pressure, technical competence, inherent safety, risk controls and human factors.
